Northwest Eye Clinic is contracted with most insurance companies. Please bring a copy of your most recently issued insurance card and billing information. We are able to bill your insurance company directly with the appropriate information.

Non-Contracted Plans
We will bill any insurance company if a patient can present us with their insurance card containing a billing claims address and if the insurance plan is not included in the above list of plans we do not accept. Some non-contracted plans will not cover all services or will pay at a lesser rate if the physician is an out of network provider. In both of these situations, patients may be responsible for the remaining balance.
